Wait, the form refers to the cadet's mother as his "non-birthing parent?" I wonder what exactly they think it takes to be considered a birthing parent if pushing the child out of her vagina isn't enough?
 
Maybe c section so the doctor pulling the baby out is the birther? Ya never know, they've gone bananas
